Please complete the referral form – a deposit of £500 will be payable to submit this referral. The final balance will be due a week before your appointment. We will contact you within 24 hours (weekdays only) to arrange your appointments. We reserve the right to refuse to assess if we do not believe the assessment is appropriate for any reason, and in such circumstances, we will refund your deposit.
